    Predictors of Resolution of Hypertension after Adrenalectomy in Patients with Aldosterone-producing Adenoma

    Primary aldosteronism (PA) is a frequent cause of secondary hypertension and is amenable to surgical intervention when it is caused by aldosterone-producing adenoma (APA). Many patients, however, continue to require antihypertensive medications to control their blood pressure after adrenalectomy. The aim of this study was to determine the preoperative factors that predict clinical outcomes after adrenalectomy in patients with APA. We studied 27 patients (mean age 45±4 yr) who had APA and underwent unilateral adrenalectomy between December 1995 and September 2008 at our institution. Clinical and biochemical data were evaluated at baseline and after a mean follow-up of 51.8±47.0 months (range, 6-159). At the end of the follow-up, 16 patients (59.3%) were considered to experience "complete resolution" without postoperative medications, whereas 7 patients (25.9%) "improved" with medications and 4 patients (14.8%) were "uncontrolled." Three factors (≤2 antihypertensive medications [P=0.007], duration of hypertension <6 yr [P=0.002], and serum aldosterone <350 pg/mL [P<0.001]) were the predictive for complete resolution in univariate analysis. Multivariate regression analysis showed that serum aldosterone level (<350 pg/mL) was the single most important factor that predicted complete resolution after surgery (P<0.001). The best preoperative clinical factor that predicted resolution of postoperative hypertension after adrenalectomy is serum aldosterone level (<350 pg/mL)

    Average R\'{e}nyi Entropy of a Subsystem in Random Pure State

    In this paper we examine the average R\'{e}nyi entropy SαS_{\alpha} of a subsystem AA when the whole composite system ABAB is a random pure state. We assume that the Hilbert space dimensions of AA and ABAB are mm and mnm n respectively. First, we compute the average R\'{e}nyi entropy analytically for m=α=2m = \alpha = 2. We compare this analytical result with the approximate average R\'{e}nyi entropy, which is shown to be very close. For general case we compute the average of the approximate R\'{e}nyi entropy S~α(m,n)\widetilde{S}_{\alpha} (m,n) analytically. When 1n1 \ll n, S~α(m,n)\widetilde{S}_{\alpha} (m,n) reduces to lnmα2n(mm1)\ln m - \frac{\alpha}{2 n} (m - m^{-1}), which is in agreement with the asymptotic expression of the average von Neumann entropy. Based on the analytic result of S~α(m,n)\widetilde{S}_{\alpha} (m,n) we plot the lnm\ln m-dependence of the quantum information derived from S~α(m,n)\widetilde{S}_{\alpha} (m,n). It is remarkable to note that the nearly vanishing region of the information becomes shorten with increasing α\alpha, and eventually disappears in the limit of α\alpha \rightarrow \infty.     Meta-analysis of Complementary and Alternative Intervention on Menstrual Distress

    PURPOSE: This study was to analyze the effect size of complementary and alternative intervention studies in reference to dysmenorrhea and menstrual distress. METHODS: In order to conduct a meta-analysis, a total of 393 studies were retrieved from the database. Twenty-eight studies that were published from March 2001 to February 2011 were selected. RESULTS: Intervention studies included seven studies on aromatherapy, five on auriculotherapy, three on each Koryo-Sooji-Chim and moxibustion, two on each heat therapy and magnetic therapy and six on other therapy. The effect size of the intervention studies on dysmenorrhea and menstrual distress was greater than 0.48 for Koryo-Sooji-Chim, moxibustion, aromatherapy, auriculotherapy and other therapy. CONCLUSION: This study suggests that drug free therapy can reduce the levels of menstrual distress, despite the small number of intervention studies and randomized controlled trials

    The Socialist Camp’s North Korean Medical Support and Exchange (1945-1958): Between Learning from the Soviet Union and Independent Course

    This study focused on the socialist camp’s North Korean medical support and its effects on North Korean medical field from liberation to 1958. Except for the Soviet assistance from liberation to the Korean War, existing studies mainly have paid attention to the ‘autonomous’ growth of the North Korean medical field. The studies on the medical support of the Eastern European countries during the Korean War have only focused on one-sided support and neglected the interactions with the North Korean medical field. Failing in utilizing the materials produced in North Korea has led to the omission of detailed circumstances of providing support. Since the review of China’s support and the North Korea-China medical exchanges has been concentrated in the period after the mid-1950s, the impacts of China’s medical support on North Korea during the Korean War period and the post-war recovery period have not been taken into account. In terms of these limitations, this study examined the medical activities by the Socialist camp of the Eastern European countries in North Korea after the Korean War. The medical aid teams from Hungary, Romania, Bulgaria, Czechoslovakia, Poland, and East Germany that came to North Korea in the wake of the Korean War continued to stay in North Korea after the war to build hospitals and train medical personnel. In the hospitals operated by these countries, cooperative medical care with North Korean medical personnel and medical technology education were conducted. Moreover, medical teams from each country in North Korea held seminars and conferences and exchanged knowledge with the North Korean medical field staffs. These activities by the Socialist countries in North Korea provided the North Korean medical personnel with the opportunity to directly experience the medical technology of each country. China’s support was crucial to North Korea’s ‘rediscovery’ of Korean medicine in the mid-1950s. After the Korean War, North Korea began to apply the Chinese-Western medicine integration policy, which was performed in China at that time, to the North Korean health care field through China’s medical support and exchanges. In other words, China’s emphasis on Chinese medicine and the integration of the Chinese-Western medicine were presented as one of the directions for medical development of North Korea in the 1950s, and the experiences of China in this process convinced North Korea that Korean medicine policy was appropriate. The decision-makers of the North Korean medical policies, who returned to North Korea after studying abroad in China at that time, actively introduced the experiences from China and constantly sought to learn about them. This study identified that a variety of external stimuli had complex impacts on the North Korean medical field in the gap between ‘Soviet learning’ in the late 1940s and the ‘autonomous’ medical development since the 1960s. The North Korean medical field was formed not by the unilateral or dominant influences of a single nation but by the stimulation from many nations and the various interactions in the process

    Dual quadratic differentials and entire minimal graphs in Heisenberg space

    We define holomorphic quadratic differentials for spacelike surfaces with constant mean curvature in the Lorentzian homogeneous spaces L(κ,τ)\mathbb{L}(\kappa,\tau) with isometry group of dimension 4, which are dual to the Abresch-Rosenberg differentials in the Riemannian counterparts E(κ,τ)\mathbb{E}(\kappa,\tau), and obtain some consequences. On the one hand, we give a very short proof of the Bernstein problem in Heisenberg space, and provide a geometric description of the family of entire graphs sharing the same differential in terms of a 2-parameter conformal deformation. On the other hand, we prove that entire minimal graphs in Heisenberg space have negative Gauss curvature.Comment: 19 page
